Below is a list of commonly used forms. Many of these forms can be downloaded for digital submission, however, some require a physical signature. Completed forms can be scanned and emailed to adventureclub@missouri.edu.

2020-2021 Adventure Club Calendar of Events
Immunization Consent
Required for all newly enrolled children; gives Columbia Public Schools permission to release your child’s immunization records to our office.
Individualized Care Plan
Required by any child that takes prescription medications, has asthma, ADHD/ADD/ODD etc., and requires a signature by your child’s physician.
Medication Authorization
This form gives Adventure Club permission to administer medications while your child is in our care.
MU Payroll Deduction – (currently unavailable)
For use by University of Missouri employees only; deductions are post tax and deducted ahead of tuition due dates.
